Remember to bring these items with you to EVERY visit:

•Insurance Card

•Driver’s License

•New or Established Patient Registration Form

•All current medications including prescriptions, herbal supplements, vitamins or, alternatively, a COMPLETE medicine list including drug name, dosage level, and frequency is taken 

 


Missed Appointments

We understand that there are times when you must miss an appointment due to emergencies or obligations for work or family. However, when you do not call to cancel an appointment with advance notice, you may be preventing another patient from getting much-needed treatment.

Conversely, the situation may arise where another patient fails to cancel and we are unable to schedule you for a visit, due to a seemingly “full” appointment book.

If an appointment is not canceled at least 24 hours in advance you will be charged for the appointment.
 


Calling Our Office

You may reach our Cool Springs office at (615) 376-8195 and our Franklin office at (615) 435-3854. To ensure you and your family receive help promptly our telephone operator will ask whether you are calling for an appointment or for advice. Our staff answers and directs your calls as quickly as possible based on the information you provide.

For An Appointment

Our physicians see patients of all ages by appointment for:

•Initial presentation with a specific illness or complaint
•Well-Child Visits
•Well-Adult Visits
•Return visits for follow-up care on a specific illness or condition •Consultations such as pre-operative evaluations

To ensure the greatest continuity of your care, our scheduling specialist will make every effort to schedule an appointment with your regular physician. If your regular physician is unavailable on the day you are seeking an appointment you have the option to see another available physician within our practice. If you feel an extended discussion is needed to address a serious yet non-urgent concern, please call in advance.


In an Emergency

Urgent calls requiring the attention of a physician are returned as soon as possible. However, if you or your child is experiencing a life-threatening situation call 911 immediately.

Urgent, Non-Emergencies that Can't Wait for An Appointment

Come to our Walk-In Clinic! Our Walk-In Clinic services are available on a non-appointment basis. When you come to the clinic the first available practitioner will see you. All patients are welcome, even if you are not an established patient with Cool Springs Internal Medicine & Pediatrics (CSIMP).

Established CSIMP Patients With Urgent Needs

Urgent and sick calls are handled by our triage nurses who are trained to help you with information and to answer questions. If you reach their voicemail, please leave a detailed description of your problem or question. Messages are checked frequently and your call will be returned as soon as possible. You can help this process go more smoothly by having on hand a pencil and paper to take notes and also the name, location and telephone number of your pharmacy. If your call must be handled by a physician or the doctor’s nurse, please leave a detailed message. Your physician or the nurse will return your call as time permits.

Urgent After-Hours Calls

After 7pm Monday-Thursdays, after 6pm on Fridays and on weekends, clinic lines are forwarded to the physician on call. A physician is on call to handle urgent calls only. Calls regarding routine medical matters and prescription refills should be handled during regular business hours. Remember, we are open on Saturday from 8am-1pm and Sunday from 8am-1pm for urgent care visits!


Prescription Refills

In order to have your prescription refill made in the most efficient manner, CSIMP strongly recommends that you call your pharmacy and ask them to fax a refill request to our office (fax: 376-2601). Be prepared by having the following information for the drug you need refilled:

•Name
•Dosage level
•Frequency taken

Please plan ahead and allow at least 72 hours for refills to be made. If after hours, we ask that you do not page the doctor on call — please call back during regular office hours. Prescriptions for new medications are generally not given over the telephone.
